In our hospital, w have processes set in place to ensure patients safety. Nvertheless, eployees will at times look for short cuts to bypass these processes in order to get work done more quickly or satisfy their supervisors. Oe case in particular gained national attention when a nurse in the operating room refused to speak up while witnessing a surgeon amputate the wrong leg because the nurse feared the wrath from the angry surgeon. Al managers attended Just Culture training hence they know what is right and what to do to keep the patients safe, engage in risky behaviors because they do not believe that they are putting patients’ lives at risk.

Al managers received Just Culture Algorithm during training whose aim was to give them guidelines or act as a tool to assist them in dealing with employees who make errors. Yt during my interview for the previous paper with middle managers, Ifound that only less than a handful of them are using it. Te managers, i this case the rider, flly believe in Just Culture and feel that we need to to Culture if we want to respond to the Institute of Medicine statistics.

Hwever, tey are reluctant or rather ignorant to change their ways of punishing employees when they commit mistakes. Te managers maintains believe that if they do not respond harshly to errors, eployees will not change and, wrst yet, oher employees will think they can get away with it too. Dring my interviews with middle managers, tey are still punitive and do not use the algorithm explaining why only 20 percent of staff feel it is to report acording to Patient Safety Culture survey that we distribute annually for the past four years.

Te Rider, abeit correctly, i not convinced that the manager will practice Just Culture. Terefore, tey will not come forward to report errors hence the organization will not. ..
